The Alzheimer's Association Rochester & Finger Lakes Region Chapter provides comprehensive guidance, support, and education for individuals living with dementia and their care partners. Families can access a free 24/7 Helpline, one-on-one care consultations, educational workshops, and peer support groups tailored to every stage of memory loss. Center for Disability Rights (CDR)

The Center for Disability Rights (CDR) is a non-profit, community-based organization in Rochester, NY, and a national leader in the Independent Living movement. CDR is run by and for people with disabilities, and its mission is to work for the full integration, independence, and civil rights of all people with disabilities. They provide a range of supportive services, including Consumer Directed Personal Assistance Services (CDPAS), skills training, peer counseling, and systems advocacy.

Office for the Aging

The Monroe and Wayne County Offices for the Aging (including Wayne County's Department of Aging and Youth) are the designated Area Agencies on Aging (AAA) responsible for planning, coordinating, and advocating for services for residents age 60 and over and their caregivers. They serve as a vital link to long-term services and support, providing unbiased information, counseling (such as Medicare/HIICAP), home-delivered and congregate meals, transportation assistance, legal services, and caregiver support programs. The Jewish Home Adult Day Health Care (part of Jewish Senior Life) is one of the most extensive medical adult day programs in the greater Rochester area. It serves older adults with physical, psychosocial, and memory needs and provides essential services such as skilled nursing, medication assistance, physical and occupational therapy, personal care, and social activities. ElderONE | Rochester Regional Health (PACE Program)

ElderONE provides comprehensive day center care designed to help older adults maintain their independence and safely remain in their own homes for as long as possible. Their specialized PACE (Program of All-Inclusive Care for the Elderly) Centers offer a collaborative, all-in-one environment combining on-site medical care, physical rehabilitation, personal hygiene assistance, and nutritional counseling. Participants also benefit from coordinated transportation, daily meals, and robust recreational therapies—including interactive movement technologies—that foster social, cognitive, and physical well-being.

HEAP (Home Energy Assistance Program) | NYS OTDA

The New York State Home Energy Assistance Program (HEAP) is a federally funded program that helps income-eligible New Yorkers manage their home heating, utility, and cooling expenses. The program provides regular seasonal heating grants, emergency energy assistance to prevent utility disconnects, and funding for heating equipment repair or replacement. Additionally, HEAP offers a summer cooling assistance component to help qualified households acquire air conditioners or fans. Specialists provide personalized options counseling and direct referrals for in-home care, transportation, home-delivered meals, respite care, and health insurance navigation. Locally operated through county offices, NY Connects simplifies the process of finding and accessing essential long-term care solutions.

 

SNAP (Supplemental Nutrition Assistance Program) | NYS Food Assistance

The Supplemental Nutrition Assistance Program (SNAP) provides monthly financial benefits to help eligible New Yorkers stretch their food budgets and access nutritious food.
